Current and Former Students applying to law schools, professional schools, state board agencies such as the bar, or federal government positions may be required to submit a request for a disciplinary check. These checks are intended to verify you have not violated any University policies or are subject to any disciplinary actions or proceedings with USC.
Requests from current or former students to release disciplinary information should be submitted by emailing disciplinarycheck@usc.edu. Please attach the form from the requesting agency with your request. Request can take up to 3 to 5 business days to complete.
Please include in your email:
- Your full name while attending USC
- USC ID number
- Phone number and email address
- Name(s) and address(es) of the Institutions that should be sent Dean’s Certification
- The signed form granting permission to release your information or a statement saying you authorize permission for the Office of Community Expectations to release the requested information.
